Even in the midst of the nation’s worst overdose crisis, 95% of people with SUD are not receiving specialty treatment because they don’t recognize a need for it. The traditional specialty SUD treatment model is structured with high barriers to care that can discourage a large number of people with SUD. SUD is widely recognized as a chronic medical condition, and yet traditional treatment models continue to require individuals already be in maintenance (abstinent) and punish a return to symptoms characteristic of chronic conditions (lapsing or returning to substance use).  By using payment reform incentives focused on system design, optimizing outreach and engagement, and integrating harm reduction and treatment, this workshop will describe how Los Angeles County is broadening the reach of SUD services to support patients with non-abstinent goals of care and provide a blueprint for health systems to better reach the 95%.
